• NHS South West London Integrated Care Board (SWL ICB) is undertaking pre-market engagement to inform the potential commissioning of a targeted prevention and empowerment service for Central Croydon. The programme is designed to address key health inequalities identified through locality population health analysis and will focus on interventions that improve access to healthcare and address wider determinants of health. The proposed service consists of three targeted prevention programmes aligned to the differing population needs of Central East and Central West Croydon: • Central East Croydon - Digital Inclusion and NHS App Uptake Programme Supporting residents aged 40 years and over who experience digital barriers to accessing health services and who are not currently registered with the NHS App. The programme will provide targeted outreach and personalised support to enable residents to access NHS digital services, improve self-management of long-term conditions and strengthen access to healthcare. • Central West Croydon - Welfare, Financial and Housing Advice Programme Providing structured casework support for adults living in IMD 1-3 areas who experience financial or housing insecurity and whose wellbeing is significantly affected by these pressures.
