Description : Nottingham City Council are seeking suitably qualified and experienced providers to deliver the Locally Commissioned Public Health Services (LCPHS) NHS Health Check programme to eligible citizens living within Nottingham City Boundaries.The National NHS Health Check programme aims to help prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia. Everyone between the ages of 40 and 74, who has not already been diagnosed with one of these conditions, will be invited (once every five years) to have a check to assess their risk and receive support and advice to help them reduce or manage that risk. Providers will be expected to produce a list of patients who are eligible for the NHS Health Check programme, using the NHS Nottingham & Nottinghamshire Integrated Care Board's eHealthscope Health Check tool, or an alternative compatible IT solution.Financial payment for the programme will be activity based as follows.• Providers will be paid £45.00 per NHS Health Check completed for an eligible priority group patient.• Providers will be paid £10.00 per NHS Health Check completed for an eligible non-priority group patient. • A bonus payment for first invitations sent to at least 70% of ...
