Description : NHS North West London ICB requires the provision of an integrated consultant-led community respiratory service in Hounslow. The ICB is looking to tender this opportunity via a closed below threshold procedure via the Atamis e-procurement portal which will be launched in the next few weeks.Key requirements include provision of a:-Pulmonary Rehabilitation Service-Home Oxygen Assessment Service-Community-based specialist Nursing & Consultant Service-Chest RehabilitationObjectives of the service are to:1. Provide an integrated care service where the Provider will work with other services/stakeholders, to ensure continuity of care is provided to the patients with chronic respiratory conditions, e.g. Chronic Obstructive Pulmonary Disease (COPD)2. Ensure pulmonary rehabilitation is accessible to all individuals with chronic respiratory conditions e.g. COPD3.
