Abstract: A Discharge To Assess Bed Primary Care Cover Service in Plymouth to provide safe, effective and agile Primary Medical care to patients who are in Bedded Intermediate Care.The care provided will be of equivalent scope as would be provided by a patient's own GP practice and indicatively include: • the personalised, Primary Medical care and continuing nature of the care of individuals • the physical, psychological and social aspects of diagnosis• the early diagnosis, initial decisions and continuing management of problems• acute, chronic and terminal types of illness• treatment, prevention and education for promoting healthThis procurement is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners
