Description:
LBH Joint Commissioning is seeking to commission specialist homeless and mental health supported accommodation services for vulnerable adults in Hounslow. The Service will support Clients with a range of complex needs with the aim of moving on to live independently in the community. These services will provide vital support and accommodation for Hounslow residents with complex needs who are homeless, at risk of crisis and/or need step down from a higher level of mental health support, with the aim of supporting them to move on to live as independently as possible in the community. The Short Stay Scheme will deliver timely assessment of needs and move on to the most suitable offer of accommodation. The services will operate across a number of different buildings in Hounslow and will be split into separate Lots. The services provide 120 units and a small number of rapid access bed spaces. • Lot One - Mental health needs supported housing - 34 units across two buildings - for people stepping down from higher needs mental health placements. • Lot Two - Homeless supported housing and Short Stay Scheme Single homeless supported housing - 75 units across nine buildings - supporting homeless adults with a range of needs, including mental health and substance use. Short Stay Scheme - 11 units across one building - accommodation providing assessment of needs and the most suitable offer of accommodation and short-stay rapid access.
