Abstract: Service SummaryLewisham has a local acute anticoagulation service at University Hospital Lewisham, which manages complex/unstable patients. A community anticoagulation service has been in operation seeing patients stable on the oral anticoagulant warfarin. The requirement of a community anticoagulation service is being amended to reflect innovation and changes to anticoagulation therapy since an initial service had launched. A newly procured Community Anticoagulation Service is to provide a clinic and housebound service to Lewisham residents which monitors warfarin, initiates Direct Oral Anticoagulants and switch patients between anticoagulants as clinically appropriate, and as per specification, working closely with healthcare providers across Primary and Secondary care in Lewisham.A detailed service specification can be downloaded from the EOI advert, following registration on ProContract via the following link;https://procontract.due-north.com/Advert/Index?advertId=66f536c3-8ac2-ed11-8120-005056b64545
