Abstract: This service aims to offer residents with a disability and their carers, of all ages:a new way to connect with community groups and local activitiesaccess to information, advice and support that enables them to overcome barriers/ challenges and live fulfilled and active livessupport to develop and maintain independence and resilience, preventing needs from escalating.The ambition is for the service to create an environment that empowers people with a disability and their carers through advocacy, self-advocacy and support that builds resilience and promotes inclusion in local community life.The contracting authority wishes to engage with potential and suitable organisations that can connect with, lead and co-ordinate local voluntary sector organisations in providing a service which directly supports vulnerable residents.
