Description:
The NHS Health Check programme is aimed at detecting those most at risk of Cardiovascular Disease (CVD) and will be offered to patients aged 40-74 years old.

This programme is provided by GPs in Leicester City who are given a payment for every patient seen and if a patient is placed on a management plan a further payment is given.

A patient who is assessed as not needing to be placed on a management plan, then the patient is recalled to have a further check every 5 years until either they are ineligible through age (or in the intervening time placed on a disease register for a condition).
In the event that a patient is assessed as needing to be placed on a clinical pathway to have their conditioned managed then a one-off payment is given to the GPs, and the patient is no longer invited for a Health Check.

This proposal is to continue with the commissioning of this service through Direct Award Process B under the Provider Selection Regime. Full detailed requirements can be accessed through the published suite of documents.

Suppliers participating will be required to register on the central digital platform (CDP). Suppliers must share their unique identifier with the Council. It is free to register and further detail on how to register can be found in the 'Central Digital Platform - Guide for Suppliers'.
