Description : Bath and North East Somerset, Swindon and Wiltshire Integrated Care Board ("BSW ICB") are seeking to commission a provider to carry out Continuing Healthcare ('CHC') Assessments for approximately 150 individuals. The assessments are a variety of those triggered by a positive CHC Checklist for all client groups, with a significant number being individuals with a diagnosed learning disability. There are also a number of individuals in receipt of CHC Fast Track funding who require the consideration of CHC eligibility to determine their ongoing funding stream. The assessments will include the completion of a Decision Support Tool to cover the period from the positive CHC Checklist to the current day. The successful provider will comply with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (2022 revised).
